Evolution of Euthanasia by Jessica French: Forms

Overview

  • Every state allows for passive involuntary and voluntary euthanasia (see definitions on the "Home" page), where a person can plan in advance to withdraw or prevent life sustaining in the case of terminal illness. In this guide, you will see Oklahoma as the prime example of where to find forms related to end-of-life planning.

  • Currently, four states allow for active voluntary euthanasia.  Oregon was the first state to enact a Death with Dignity Act and has had the most advancements in the laws concerning this area of law.  Oregon is the model for current states that allow active voluntary euthanasia and for future states who have been debating over whether to legalize physician-assisted suicide.

  • See "Physician and Patient Forms" under "Legal Resources."
