§ 67:81. Checklist—Action to recover for sickness or disease under health insurance policy
1. Authority of insurer to issue health insurance policies.
2. Issuance of insurance policy.
3. Consideration for issuance of insurance policy.
4. Terms of policy, either in substance or by attaching copy of policy to complaint.
5. Description of sickness or disease covered by policy and suffered by insured.
6. Specific losses suffered by insured as result of sickness or disease.
7. Compliance with conditions precedent of policy by insured, or waiver of conditions by insurer.
8. Giving of notice and proof of loss, if required.
9. Failure of insurer to pay loss as provided by health insurance policy.
10. Action filed more than 60 days after submission to insurer of written proof of loss as required by the policy [See FS § 627.616].
